Existing Patients
Patient Registration Paperwork
ALL patients must fill out new patient registration paperwork prior to their first visit. This information is necessary in order for us to refill medications, obtain referrals, order lab work, and bill insurance.
Prescription Refills
For refills, please contact your pharmacy and have them request medication refills. Alternatively, this can be done through the patient portal (preferred) or by calling the office and leaving a message with the name, dosage and directions of all medications needing to be refilled, along with your pharmacy information. Please do not send duplicate requests.
Appointments
We encourage patients to come in for chronic disease management appointments, annual exams/physicals, and sick visits and not delay their medical care.

We are seeing most patients in office as we feel that this provides the best and highest level of care.
However, please inform the office if you are ill (fever, chills, cough, shortness of breath, diarrhea) PRIOR to coming into the office. Please wear a mask as well. We will take special precautions to keep you and our staff safe.
Medical Records
In order to get previous medical records, we need a signed release form. You can find our Medical release form under our Patient Forms tab to send to the previous practice in order to obtain records.
Referrals
In order to obtain referrals, we must have your patient registration paperwork (specifically insurance information), along with the name of the physician you are seeing, the location of the clinic and the reason for the visit.
